Dementia: for healthcare professionals

GPs and primary care teams play a crucial role in ensuring all their patients living with Dementia and their carers have a diagnosis recognised and support needs met.

Despite the challenges of modern day primary care there are opportunities for GPs to use IT resources to understand their local populations, target at-risk groups and use proven clinical tools to ensure high levels of dementia diagnosis and consistent approaches to clinical care that reach across the Dementia and Frailty work streams.

The Yorkshire and Humber Dementia Clinical network has produced a range of tools and resources to help GPs identify, diagnose and support people living with dementia.

Meanwhile, people whose lives have been affected by dementia, their families and carers can get free help and support from a new service available across North Yorkshire.

The service is being provided by Dementia Forward and is funded by North Yorkshire County Council and the clinical commissioning groups (CCGs) covering the county, as part of their work to help people live independent, healthy lives for as long as possible.

It is recognised as a vital source of information, advice and support for anybody affected by dementia across the county and has developed a comprehensive range of services; all with people living with dementia at their heart. Dementia Forward runs a Helpline for anyone looking for support and advice and has a team of local Dementia Support Advisors who will provide confidential advice and information that will help you to:

  • Understand dementia and what might happen in the future
  • Choose the types of services and support that best meet your needs
  • Stay connected to your local community by supporting you to take part in a range of wellbeing services
  • Make informed decisions about your care, treatment, wellbeing and support needs
