A free service lasting up to 24 weeks to support adults to lose weight, be more active and maintain a long-term healthy weight

The adult weight management service is fully funded by North Yorkshire County Council public health and is available across North Yorkshire.

  • The initial programme includes:
  • nutritional sessions;
  • weekly weigh-ins;
  • physical activity sessions;
  • techniques to support behaviour change;
  • a personal weight loss plan;
  • a physical activity agreement; and
  • regular reviews to monitor progress. 

The overarching aim for each individual is to gain control of their own weight and health.

If the individual is successful in achieving a five per cent weight loss after the first 12 weeks, they will be eligible for another 12 weeks' worth of free support in the form of a maintenance programme.

If the individual is unsuccessful in completing a 12-week programme or achieving a five per cent weight loss, they can be offered a re-referral after a 24 week gap.

Those eligible for the service
Individuals who are eligible to access the adult weight management service must:

  • be aged eighteen or over;
  • have a BMI equal to or greater than 25; and
  • live in North Yorkshire, be registered with a GP practice in North Yorkshire, or be working for an organisation based in North Yorkshire.

Those not eligible for the service
Individuals meeting the following criteria are excluded from this service: 

  • those under the age of eighteen;
  • who have a BMI of less than 25;
  • who are pregnant or breastfeeding;
  • have a diagnosed eating disorder;
  • have an underlying medical cause for obesity and would benefit from more intensive clinical management from a tier three service;
  • have a significant unmanaged co-morbidity* or complex needs as identified by their GP or other healthcare professional; or
  • have had bariatric surgery in the last two years.

*e.g. type 2 diabetes, cardio vascular disease, chronic obstructive pulmonary disease (unmanaged meaning not on medication and / or not subject to regular clinical review, or not completed a management programme such as diabetes management or cardiac rehabilitation).

Those adults that do not meet the eligibility criteria can still receive weight management support from NHS Choices.

Making a self-referral

If you would like to make a self-referral to the adult weight management service, please contact the provider for your district area.

Once the referral is received, the provider will contact the individual to arrange an initial triage assessment.

Healthy Choices

Healthy Choices is a bespoke family service aimed at providing a fun and engaging way of learning about healthy living and is delivered by a team of Healthy Lifestyle Advisors based across North Yorkshire. The team works with families to make positive, realistic and achievable changes to their lifestyle suitable to their individual circumstances.

Healthy Choices want to provide families with the knowledge and tools to confidently take control of their health and opt for a healthier happier way of living.

Am I eligible?

Health Choices is aimed at 5-19 year olds living in North Yorkshire that fall above a healthy weight (91st centile on BMI Chart).

What will I get?

 An initial assessment will be completed for each family which will look at a range of lifestyle behaviours including diet and sedentary behaviour alongside confidence and self-esteem.

A lifestyle advisor will then work with each family for 12 weeks, ten of these weeks will be based around their core curriculum and two weeks will allow for flexible bespoke sessions, which will be tailored for each family.

On completion of the 12 weeks further support will be offered through regular phone calls or if felt necessary additional one to one catch ups, each family will also be offered follow up visits at 6, 9 and 12 months.

Referrals may come from both professional and self-referral routes.
